Changes in the concentration of either sodium or calcium ions inside or outside the cell affect the direction and magnitude of sodium-calcium exchange. Under normal conditions, the low intracellular sodium concentration favors sodium influx and clacium efflux. Some drugs take advantage of the functional coupling etween the sodium pump and the sodium-calcium exchanger to exert their effect as positive inotropes.
